2.15 Transcripts and Enrollment Confirmation Statements

HMS students may request complete transcripts of their own records or statements certifying their registration at HMS at the Registrar’s Office.

Requests for transcripts must be made in writing with an original signature to the HMS Registrar via the request form

Please allow 2-5 business days for processing a request. Processing time may vary depending on the time of year and volume of requests; therefore, submitting the request(s) 1-2 weeks in advance of your deadline is advisable.
