HMS students may request complete transcripts of their own records or statements certifying their registration at HMS at the Registrar’s Office.

Requests for transcripts must be made in writing with an original signature to the HMS  
Registrar via [the request form](https://meded.hms.harvard.edu/sites/hwpi.harvard.edu/files/hms-med-ed/files/reqform_revised_oct_2017.pdf?m=1513024311).
